APHIS Revises Lacey Act Declaration Form

The Animal and Plant Health Inspection Service has released a revised version of the Lacey Act Amendment Plant and Plant Product Declaration Form (PPQ 505) for imported plants and plant products, as well as a new Supplemental Form (PPQ 505B). Changes to the Lacey Declaration form include that the HTS number and entered value are now in a table, on a line-item basis, along with the article/component and plant scientific name, instead of each formatted as a single field. The new form also requires the preparer to provide his phone number on the certification line. Additional alterations including changing the form to a horizontal format, decreasing the number of rows in the table from 16 to 5, and containing the entire PPQ 505 form on a single page. There is now a supplemental form (PPQ 505B), which is intended for use if additional rows are needed. The supplemental form requires the entry number, and consists of a 12 row table for recording additional articles or components. The revised form PPQ 505 also provides new declaration examples for the two fields that were moved to the table, and provides instructions regarding entering container numbers in field three. According to the instructions, should a shipment contain more than one container number, the filer should check the “see attachment” box and list all of the containers on a separate sheet attached to the PPQ 505 form.
